RESEARCH WORK
Basic and applied research are realized on the basis of inter-chair laboratory, Institute of Molecular Medicine and patobiochemistry, branch/Collaborating Centre of the Scientific Center of Neurology (Moscow), the Russian-Japanese Center of Microbiology, Epidemiology and Infectious Diseases, the Russian-German Centre of respiratory support and critical care medicine, Russian-French associate laboratory “Joint evolution of humans and the environment in Eastern Siberia”, Russian-Canadian laboratory biomolecular technologies in the framework of the Russian-Japanese partnership in the field of neuroscience.
The organization and coordination of research attended the University Academic Council, 10 problem committees, the Central Coordinating Council for Science, postgraduate and doctoral studies department, dissertation councils, department of clinical studies, local ethical committee and other scientific KrasSMU units.
In 2014, protected employees, doctoral students, post-graduate students of the university 16 doctoral and 22 master’s theses.
In KrasSMU operates 3 specialized dissertation council for doctoral and master’s theses on 9 specialties. Published by the university scientific and practical journal “Siberian Medical Review” is included in the list of the RF HAC journals. The scientific work developed in the framework of traditionally established scientific schools in the main areas of training.
STAGES OF FORMATION AND DEVELOPMENT PROSPECTS OF ARRHYTHMOLOGY
Arrhythmology as science almost began to develop only 4 decades ago. This, incidentally, is one of the reasons for our choice of the main directions of research in the mid-70s. For certain 4 decades have seen rapid and sometimes Dramaten-nomic development of this branch of medicine. There is probably another area of medicine, koto-paradise would have so quickly absorbed the achievements of modern electronics and so productive that would be used to achieve this, not only for diagnosis but also for therapy. At the same time, there is a field of medicine in which the pharmacological boom of recent decades would have ended in such disappointment and dramatic reappraisal of values.
The most rapid and impressive progress arrhythmology due primarily to two factors:
1. The beginnings in 60s Clinical Electrophysiology (EPS) of the heart.
2. The introduction and rapid improvement of implantable pacemakers (EKS) and surgery on the pathways of the heart
3. The development of the study of genetic aspects of arrhythmias.
In the late 70s and early 80s of the last century, the Scientific School in Krasnoyarsk began to form in the field of arrhythmology. The study was based on Professor F. F. Kostiuk. Fyodor Filippovich did not interfere in the details of the investigation. But his good knowledge of world literature and critical attitude made us raise the bar for research and keep it up to the mark.
Fyodor Philippovich managed to create a creative atmosphere led by him by a graph lektiv. Once they asked why he “suffers” and even supports people who criticize his ideas, criticize them, including the public. By the way, many of his ideas, which were initially perceived by our “hostility” in the future, are already perceived as the only true ones. To the question, Fedor Filipppovich said the words of Bismarck: “Relying only on what is resisting.” These words were not immediately understood. But now, with age, Stan becomes clear that this answer has a deeper meaning. This is one of the most important principles of scientific school.
The founder of the Krasnoyarsk Scientific School “Problems of the etiology, diagnosis, and treatment of cardiac arrhythmias,” is a professor emeritus at the State Krasnoyarsk Medical University, honorary cardiologist of Russia, the pro-Russian Honored Doctor Professor V.A. Shulman companionship more than 30 years ago, there was a very hard-working, friendly creative team at the clinic the 20th city hospital. This young doctor S. Nikulin. G.V.Matyushin, S.V. Salzberg (now known arrhythmologist in Germany), T. Y. Bolshakov, B.V. Nazarov and others were selected while a little-studied topic: “Pathology sinus node and the lower parts of the conducting system of the heart. ” new methods of diagnosis and treatment of cardiac arrhythmias have been introduced.
Pathology Study of the cardiac conduction system, the identification of familial cases of this pathology has drawn attention to the genetics of arrhythmias, while one of the least-studied the issues identified arrhythmology. This question has been charged with the young medical residents S. Nikulin. At the same time cooperation with the Institute of Medical Genetics, Tomsk Research Center was established. Director of the Research Institute of the academician VP Puzyrev became one of the supervisors of Ph.D. and doctoral theses in a subsequent S. Nikulin. Ph.D. thesis S. Nikulin was devoted to genealogy and genetics, and his doctoral thesis were studied problems of genetics intracardiac blockades. In these works, we were first studied phenotypic predictors of cardiac conduction system disease. This part of the work carried out under the supervision of Professor V.G. Nikolaev. The type of constitution (somatic) was taken as a model of the phenotype. This approach allows us to anticipate the disease and thus to solve the issues of early diagnostics.
Postdoctoral S. Nikulin and G.V. Matyushin clinic continued development areas: “The genetics of cardiac arrhythmias,” close cooperation with the Research Institute of Internal Medicine and Director of the Institute corresponding member of the Academy of Medical Sciences MI governor. At the same time established contacts with the Institute of Heart of Victor Chang (Sydney, Australia).
Research in the field of arrhythmias have been expanded, in particular, a detailed analysis was subjected to the genealogical and genetic aspects of atrial fibrillation. There have been reserved on the subject dissertations O.O. Isachenko (Kuznetsova), N.V. Aksyutin, K.V. Dudkina. As a result of joint work arrhythmologist monograph “Genealogy and genetics of cardiac arrhythmias,” it was published in 2006, recognized as the best work of the year in the Krasnoyarsk State University-MU.
In 2010, after the report of S. Nikulin at the Presidium of SB RAMS scientific direction “Genealogy and genetics of cardiac arrhythmias,” it was recognized as a priority for SB RAMS.
Currently, under the direction of Professors S.Y. Nikulin, and V.A. Shulma on KrasSMU in developing new scientific direction “Clinical and genetic predictors multifactorial diseases.” In particular, the completed work on his doctoral thesis and successfully defended in 2014. For the first time in the Russian Federation carried out research-myocardial infarction patients. It is shown that the identified genetic predictors of myocardial infarction are independent of traditional risk factors, however, work continues on the integration of genetic predictors in existing risk assessment scale of myocardial infarction and the creation of complex clinical and genetic risks
On the basis of clinical hospital number 20 has accumulated vast experience in the treatment of patients with acute myocardial infarction. Jointly developing the direction of “Comparative evaluation of mathematical methods for prediction of myocardial infarction” (Associate of Professors of Russian Federation) with the Research Institute for Mathematical Modelling (pro-Professor Gorban). Under the direction of master’s theses are protected S.E. Golovenkin, B. V. Nazarov, O.V. Smirnova, T.M. Parfenova.
Over the past 15 years at the clinic, about 20 international cus-cal research has been carried out. One of the participants in such studies, associate professor of the department of the employee PA Gear was the initiator and creator of the “Department of Clinical Research” in the Krasnoyarsk State Medical University. The creation of this department greatly intensified research data within the clinical departments of the University.
From 1996 to 2006 VA Shulman, while the head of the department of internal diseases №1, was the chief cardiologist of the Krasnoyarsk Territory. With his active participation prepared 2 targeted programs of the Office of Health Administration of Krasnoyarsk region: “Support for cardio-rheumatological care to the population edge”, “Pro-prevention and treatment of hypertension.” With full organizer and strategist qualities, Vladimir Abramovich has employed as the main experts of their students – T. Yu Bolshakov (main rheumatologist edge), DA Kuzhel (chief specialist in functional diagnostics edge), S. Nikulin ( chief cardiologist of the city of Krasnoyarsk). All of them are practicing in these positions for the moment. This scientific-practical tandem allows cardiology and arrhythmology edge is at a high international level.
At the initiative of the Krasnoyarsk Scientific School “Problems of the etiology, diagnosis and treatment of cardiac arrhythmias,” and its leader V. Shulman, who for the past 20 years, is chairman of the Regional Society of Cardiology and a member of the Board of the Russian Society of Cardiology in 1996, was held 1- I edge of Cardiology conference. Since that time, these forums have become a tradition. They attract a large number of doctors of the Krasnoyarsk Territory and the neighboring regions. They are actively involved leading cardiologists in Moscow, St. Petersburg and abroad.
In 2014, members of the scientific school of performing the public task of the Russian Federation Ministry of Health in the field of basic research in cardiology: “Study of polymorphisms in genes that lead to the development of cardiac arrhythmias and myocardial infarction.” The following grants were obtained:
